Steady, practical support for everyday life

Community-Based Support with Simon is for people who benefit from calm, respectful support to take part in everyday life.

Support may include getting out into the community, building routines, attending appointments, using public transport, shopping, gentle outdoor activity, or spending meaningful time in local places.

Sessions are shaped around the person’s interests, goals, energy levels and support needs. The focus is on working at the person’s pace, building trust over time, and supporting participation in ways that feel safe, useful and meaningful.

Where helpful, support can also include gardens, parks, walking trails, nature spaces or other outdoor settings.



What support may involve

Community-Based Support can be shaped around the person’s goals, routines and everyday life.

Community access

Cafés, libraries, shops, leisure centres, parks, community gardens or other places of interest.


Routines and follow-through

Weekly activities, planning, preparation and gentle support to keep things moving.


Appointments and follow-through

Support to attend appointments and follow through with strategies from allied health or other professionals, where appropriate.


Confidence and connection

Trying new activities, building comfort in community settings and participating at the person’s pace.


Everyday activities

Shopping, errands, simple planning or practical tasks in the community.


Outdoor activity

Walking, parks, nature spaces or garden-based activities where helpful.

Who this support may suit

Community-Based Support is shaped around the person’s goals, routines, interests, support needs and pace.


May suit people who need:

Calm, steady support to take part in everyday life, build routines, practise community access, attend appointments, or participate at their own pace.


Best suited to:

Community access, garden-based support, counselling-informed support work, practical participation and everyday routines.


Not currently offered

Personal care, manual handling, high-intensity personal support, nursing, complex behaviour support, crisis support, overnight support or emergency response.

Practical information

Community-Based Support may be available for private clients and self-managed or plan-managed NDIS participants, where the support aligns with the person’s goals, needs and funding.

Support can take place in the community, at home, in garden or nature settings, or alongside relevant appointments and activities.

Session length, location, travel, costs and suitability can be discussed during enquiry.

Simon is not currently an NDIS registered provider.
